Job Description:
Seeking a passionate Primary Care/Mental Health Integration LCSW
for a Community Outpatient Clinic in Bowling Green KY. Provide
direct patient care, including evaluations, therapy, crisis
intervention, and emergency response. Collaborate with
interdisciplinary teams to develop and implement comprehensive care
plans. Perform case management, advocate for veterans, and educate
on independence and empowerment. Maintain accurate documentation
and ensure confidentiality. Participate in staff meetings, program
development, and outreach activities. Meet licensure,
certification, and experience requirements, including a
master--------s in Social Work, valid LCSW license, and 1-3 years
experience with adult patients. Proficiency in EMR, strong
communication skills, and a positive attitude are essential. This
