Registered Nurse (RN) | Case Manager Location: Manitowoc, WI Agency: Cynet Health Pay: $2,746 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/2/2026 About the Position Job Title: RN Home Care - Hospice Profession: Nursing Specialty: Home Care - Hospice Duration: 13 weeks Shift: 8hr Day Shift Hours per Shift: 40 Experience: 3 years of home health experience required License: Active state PT license Certifications: BLS Must-Have: - Strong skills in start of care/admissions, evaluations, fall risk assessments, gait speed analysis, manual and orthostatic BP measurement - Competence in managing adult patients with post-ortho surgeries, generalized weakness, strokes, comorbidities (CHF, COPD, oncology, palliative, renal failure, hypertension, dementia) - Ability to provide individualized patient exercises and measure range of motion (ROM) - Experience with Oasis and EMR systems, specifically EPIC Description: This position covers a rural area with a typical average of 6 patients daily. Daily interdisciplinary meetings will be held at 8 AM. The role requires working independently, with supervision available if needed. Staff support includes a PT accessible by phone and a manager for guidance. Flexibility is essential for scheduling, including 1 Saturday every 5-6 weeks, but there are no on-call duties or holiday expectations. Individuals will need their own vehicle, a valid driver’s license, and active insurance for travel to patients' homes. Staff will be compensated for mileage incurred during travel. Scrub color is open to any solid color or business casual attire with appropriate shoes. Team dynamics are fast-paced, with tenured staff available to assist and share knowledge.
