Registered Nurse (RN) – MDS Coordinator Jefferson County, WI We are looking for an experienced and detail-oriented Registered Nurse (RN) to join our team as an MDS Coordinator in Jefferson County, WI. This is a unique opportunity to focus 100% on MDS — no floor shifts required —making it ideal for someone passionate about accuracy, compliance, and care planning. Position Highlights: Full-time RN MDS Coordinator role Dedicated MDS position – no floor responsibilities Facility with just over 100 beds Fast-paced environment serving rehab, long-term care, and hospice residents No ventilators or tracheostomy care Key Responsibilities: Complete and coordinate all MDS assessments in accordance with federal and state regulations Ensure timely and accurate submission of MDS data Collaborate with the interdisciplinary team on resident care plans Monitor case mix index (CMI) and support reimbursement optimization Maintain compliance with CMS guidelines and facility policies Qualifications: Active RN license in Wisconsin Previous MDS experience preferred Strong understanding of RAI process and MDS regulations RAC-CT certification preferred, but not required Excellent organizational, analytical, and communication skills Ability to manage deadlines in a fast-paced setting Why Join Us?
