Job Description Family Guidance Centers, Inc. (FGC), a

positive, lasting change. Essential Duties: - Processes patients
from assessment to discharge in both medications assisted treatment
and drug free treatment programming. - Dispenses all medication
under procedures set forth by Family Guidance Center, Substance Use
Prevention and Recovery, Clinical Sustainability Assessment Tool
and the Drug Enforcement Administration - Monitors the
appropriateness of methadone and medication dosages. - Monitors the
physical & mental health well-being of patients and documents
findings. - Must be available to travel if needed Schedule: Two
days per week & every other Saturday (7:15am - 10:00am); 16-24
hours per week Start Date: ASAP Work Location: 1220 S. Wood St.,
Chicago, IL Pay Rate: $27.00-$30.00 per hour Family Guidance

Experience and Skills: Education and/or Experience: At least two
years' experience in hospital or agency setting preferred
Certificates, Licenses, Registrations: L.P.N. registered in the
State of Illinois and certified in CPR required
